Please note: Images supplied as part of your subscription will be a mix of colour and monochrome (black and white).

Digital files supplied as part of a sport image subscription are for PERSONAL and social media use. You cannot do anything else with the pictures ever. Zero club use. Zero sale or resale. Zero products/merchandise. Zero derivative/AI use. Zero brand use. Zero advertising use. Zero business use. Zero sponsorship use. Nothing that isn’t listed above ever. All these additional uses require a specific licence for use.

All sport image subscriptions are subject to the sport image subscription terms and conditions.

PLAYER SELFIE – please take a selfie now. or have a selfie available to upload, with a clear image of your face – you will need to upload this image during the completion of this form; this is so I know who I am to photograph.

You have the option of a primary and secondary sport; If you wish to be photographed as part of your subscription at a secondary club/sport, for example cricket during the cricket season in addition to say football in the football season, then this is an option – please complete the applicable details in the form below.

You can be photographed whether playing in a team (ie football, rugby, hockey, cricket, basketball etc.) or as an individual sports person (ie athlete, tennis player, squash player, yachtsman etc.).

DIRECT DEBIT MANDATE

I will be in touch via email, including a request to complete the Direct Debit Mandate for your chosen sport image subscription package. You must subsequently complete this mandate to confirm your subscription.

Where you are subscribing with the £8.99 per month package (pay monthly quarterly image subscription), you will receive you high-resolution image on receipt of your third consecutive payment, and quarterly thereafter.
